Please fill out one of forms below according to your situation before your first visit. Your responses help us understand your health history, physical conditions, preferences, and goals to tailor a safe and effective massage session just for you. All information is confidential and used only for your wellness and care.

Spa Scheduling Policy:

Health & Intake Forms

Clients must complete a confidential Health Intake Form or Prenatal Massage Intake Form before the first session. Please update us on any changes to your health or medications before each visit.

All client information is confidential and will be handled in accordance with professional ethics and applicable laws.

Massage Therapy Policies:

Arrival Policy

Please arrive 10–15 minutes early to complete any necessary paperwork and begin your session on time. Late arrivals may result in a shortened session.

Late Arrival

To ensure you receive the full benefit of your session, we kindly ask that you arrive 10–15 minutes prior to your scheduled appointment time. This allows time to complete any necessary forms, use the restroom, and begin your session on time in a relaxed manner.

If you arrive late, your session may be shortened to avoid delaying the next client. Full payment will still be required for the scheduled time.

Cancellation Policy

We understand that life happens. If you need to cancel or reschedule your appointment, please provide at least 24 hours’ notice. This allows us to offer the time to another client.

Appointments canceled with less than 24 hours’ notice, or missed without notice, may be subject to a 50% cancellation fee or forfeiture of any prepaid sessions.We appreciate your understanding and cooperation

Inappropriate Behavior Policy

Professional, respectful behavior is expected at all times. Any inappropriate behavior will result in immediate termination of the session, and full payment will be required.

Illness Policy

Please reschedule if you are feeling unwell. Massage while sick can worsen symptoms and risk spreading illness. There is no fee for illness-related rescheduling with appropriate notice.

Minors

Clients under age 18 must have a Parent or guardian's consent and may require a chaperone during the session.

Payment

Payment is due at the time of service. We accept Cash, Credit/debit Cards, Venmo, and Paypal, gift certificates.

Client Behavior Policy

At our massage spa, we are committed to creating a safe, respectful, and relaxing environment for both clients and therapists. To maintain this standard, we ask that all clients adhere to the following guidelines:

  • Respectful Conduct

Treat staff and other clients with courtesy, dignity, and respect.Harassment, inappropriate language, or aggressive behavior will not be tolerated.

  • Appropriate Behavior During Services

Clients must maintain proper boundaries and follow all professional instructions during sessions. Inappropriate requests, advances, or misconduct of any kind will result in immediate termination of the session and possible permanent ban from services.
